The $2,000 cap and the Payment Plan
Starting in 2025 — and continuing in 2026 — Medicare Part D out-of-pocket drug spending is capped at $2,000 a year (indexed). Once you hit it, prescriptions are $0 the rest of the year. The optional Medicare Prescription Payment Plan lets you spread that $2,000 across 12 monthly bills instead of paying it at the pharmacy counter all at once.
